These Terms & Conditions (“Terms”) govern your access to and use of the website www.centiva.health and the assessments, programmes and related services (the “Services”) offered by Centiva Healthcare Private Limited (“Centiva Health”, “we”, “us” or “our”).
Acceptance of these Terms
By visiting the website or using the Services, you confirm that you have read, understood and agree to be bound by these Terms, our Privacy Policy and our Refund Policy. If you do not agree, please do not use the Services.
Eligibility
You must be at least 18 years old and legally able to enter into a binding contract to use the Services. By using them, you confirm that you meet these requirements. If you book on behalf of another adult, you confirm that you have their consent to share their information with us.
Our Services
Centiva Health offers preventive and precision health services, including a 360° health assessment (questionnaires, laboratory tests, body composition and imaging) and personalised health optimisation protocols reviewed by qualified physicians. Services are available in selected locations in India and may change over time. Nothing on the website guarantees that a particular service is available where you are.
Not for emergencies. Our Services support long-term health and do not replace emergency care. If you have a medical emergency, call 112 or go to the nearest hospital immediately.
Website content is for general information only and is not medical advice. Personal recommendations are given only after an assessment and physician review. Individual results vary, and we do not guarantee any specific health outcome.
Information you provide
- You agree that all information you give us is true, accurate, current and complete.
- We may cancel a booking or programme, or withhold or reverse a payment, if information provided is found to be false or misleading.
- By sharing your contact details you consent to us contacting you by phone, SMS, WhatsApp or email about your bookings, results and Services.
- You are responsible for keeping any login credentials confidential.

Bookings and appointments
- Appointment and sample-collection times are indicative and may change; we will let you know if they do.
- Some tests need preparation such as fasting. Results may be delayed or affected if preparation instructions are not followed.
- We may reschedule or cancel appointments because of physician or partner availability or other operational reasons. In that case we will offer a new time or a refund.
Payments
Prices are shown in Indian Rupees and include applicable taxes unless stated otherwise. Payments are processed by third-party payment gateways and banks, which have their own terms. A payment is complete only once the gateway confirms it with a transaction ID.
We may accept credit and debit cards, net banking, UPI, wallets and other modes we enable from time to time.
We are not responsible for failed, delayed or declined transactions caused by banks or payment providers. If your account is debited but the booking is not confirmed, please contact your bank, and let us know so we can help.
Cancellations and refunds
Cancellations, rescheduling and refunds are governed by our Refund Policy, which forms part of these Terms.
Acceptable use
You agree not to:
- use the Services for any unlawful purpose or in breach of these Terms;
- impersonate anyone or misrepresent your connection with Centiva Health;
- interfere with or attempt to gain unauthorised access to our systems; or
- copy, scrape or republish website content without our written permission.
Intellectual property
The website, the Centiva name and logo, protocols, reports and all content are owned by or licensed to Centiva Health and protected by law. You may use reports provided to you for your personal, non-commercial purposes only.
Third-party links and services
The website may link to third-party websites or rely on third-party partners. We do not control and are not responsible for their content, policies or practices.
Limitation of liability
We work to keep the website accurate and available, but it is provided “as is” and may contain errors or be interrupted. To the fullest extent permitted by law, Centiva Health is not liable for any indirect, incidental, special or consequential loss, including loss of profits or data, arising from:
- your use of, or inability to use, the website or Services;

- events beyond our reasonable control.
Our total liability for any claim relating to the Services is limited to the amount you paid for the specific Service giving rise to the claim.
Indemnity
You agree to indemnify Centiva Health, its directors, employees and partners against claims and losses arising from your breach of these Terms or misuse of the Services.
Governing law and disputes
These Terms are governed by the laws of India. Subject to applicable law, the courts at Mumbai, Maharashtra have exclusive jurisdiction over any dispute arising from them or from the Services.
General
- If any provision is found unenforceable, the rest of these Terms remain in effect.
- Our failure to enforce a provision is not a waiver of our right to do so later.
- Nothing in these Terms creates a partnership, agency or employment relationship between you and us.
- These Terms, with the policies they reference, are the entire agreement between you and Centiva Health about the Services.
Changes to these Terms
We may update these Terms from time to time. Changes take effect when published on this page, and continued use of the Services means you accept them.
